Athletic Forms

Each player will need to have a physical completed, forms signed, and uploaded to the SportsWare Online site.  Detailed information can be found by going to the Jesuit homepage and clicking on Athletic Forms link in the QuickLinks.  Here is the link if you're have trouble finding it on the Jesuit site.

Once the forms are completed you will be responsible for uploading the files to the SportsWare Online website.  A pdf file is located at the bottom of this page that will step you through the process to login and upload the files. 

Please complete the Jesuit Athletic Forms in July so that there are no delays in the process.  Remember to bring the FHSAA Pre-Participation Physical (EL-2) form with you to the doctor’s office.  That form needs to be signed by the doctor.  Once the physical is completed there are a couple of other forms and processes that must be uploaded to the SportsWare Online site. All of the instructions are located at the link that was included above. 

If you experience any issues uploading your documents to the SportsWare site you can contact Ricky, the athletic trainer, at smedicine@jesuittampa.org. Make sure to get that physical appointment scheduled as soon as possible.  The medical offices are very busy this time of year with sports physicals.
